The Future of Cataract Surgery Has Arrived: The World’s First Fully Robot-Assisted Cataract Surgery

Imagine a cataract operation where the surgeon controls a robotic system capable of making extremely precise movements inside the eye.



That may sound like science fiction, but in 2026, it became a reality.

In April 2026, ForSight Robotics announced the world’s first fully robot-assisted cataract surgery performed in a human patient, using its JASPER™ Robotic Surgical Platform. The procedure was completed from start to finish with robotic assistance, marking a significant milestone in the development of robotic eye surgery.

But what exactly does “robotic cataract surgery” mean — and does this mean a robot can now perform surgery without a surgeon?

Not quite. A robot — but still very much a surgeon in control


One of the most important things to understand about this breakthrough is that JASPER is not an independent robot surgeon.


The system uses surgeon-controlled telemanipulation.


In other words, the ophthalmologist remains in control of the operation, while the robotic platform translates the surgeon’s movements into highly precise micromovements.


JASPER combines technologies including:


  • High-definition 3D visualisation

  • Real-time eye tracking

  • Motion scaling

  • Tremor elimination

  • Advanced computer vision

  • Bimanual robotic manipulation


Together, these technologies are designed to give the surgeon enhanced precision, stability and control during delicate procedures.


So rather than replacing the surgeon, the technology is better thought of as an advanced surgical platform designed to enhance what the surgeon can do.


Why is this such a big deal?


Cataract surgery is already one of the most successful and commonly performed surgical procedures in the world.


The cataractous lens is removed through a very small incision and replaced with an artificial intraocular lens (IOL).

Modern cataract surgery is highly sophisticated, but it still requires an ophthalmologist to perform incredibly delicate movements within a very small space.


Robotic technology could potentially offer another level of precision, stability and reproducibility.


ForSight Robotics says JASPER is designed to perform the complete cataract procedure while maintaining surgeon control throughout the operation.


And the technology isn't simply being demonstrated in a laboratory.


From a futuristic concept to human surgery


The first human procedure was performed by Dr Alexey Rapoport, with Dr Robert T. Ang of the Asian Eye Institute serving as Principal Investigator.


The procedures were performed using standard cataract anaesthesia approaches rather than general anaesthesia — including topical anaesthesia or mild sedation — and patients were discharged on the same day.


Then came another remarkable milestone.


By August 2026, ForSight Robotics reported that its first-in-human experience had expanded to 26 patients, with all 26 procedures completed robotically and without conversion to manual surgery.


That is significant because it begins to demonstrate not only that robotic cataract surgery is technically possible, but that the technology may potentially be integrated into a familiar cataract surgery workflow.


Is this the beginning of autonomous eye surgery?


This is where things get particularly interesting.


The long-term vision for surgical robotics goes beyond simply assisting a surgeon.


Researchers are exploring how artificial intelligence, computer vision, advanced imaging and robotics could eventually allow surgical systems to interpret information, assist with decision-making and automate increasingly complex surgical tasks.


However, fully autonomous cataract surgery — where a robot independently plans and performs the entire operation without real-time surgeon control — is not yet the reality.


Experts in the field continue to emphasize that current robotic cataract systems require human supervision and have important limitations.


So we are not quite at the point where you can walk into an operating theatre and say:


“Computer, please remove my cataract.”


But we may be getting closer to a future where robotics becomes an increasingly important part of ophthalmic surgery.


What does this mean for patients?


For patients, the most important question isn't simply: “Can a robot perform my surgery?”


The more meaningful question is:


“Can technology help my surgeon perform surgery with greater precision, consistency and control?”


That is where robotic surgery could potentially have its greatest impact.


Importantly, JASPER is still under development and has not yet been approved for commercial use. More clinical research and regulatory evaluation will be needed before this type of technology becomes part of routine cataract surgery.


And this isn't the only step toward robotic eye surgery


The development of robotic cataract surgery is part of a much bigger technological evolution in ophthalmology.


Modern eye surgery already incorporates technologies such as:


Advanced imaging → Computer-guided planning → Femtosecond laser technology → Artificial intelligence → Robotic assistance


Each development builds on the last.

Perhaps the most exciting part of this story is that we don't yet know exactly where it will lead.


Today's robotic systems still require a highly skilled ophthalmologist.


Tomorrow's systems may incorporate increasingly sophisticated artificial intelligence, imaging and automation.


And somewhere between the two lies the future of ophthalmic surgery.


The robot isn't replacing the surgeon.

It is helping redefine what the surgeon may be capable of.
