Please bring any previous eye test results, a list of medications & history of previous eye surgeries. If you have received our online form to complete via email or SMS, please complete the form before your appointment.

No. Dr. Deist is not contracted to medical aid or medical insurances. Your bill must be settled with Dr. Deist on the day. You will however be provided an invoice after your consultation/surgery. This invoice may then be forwarded to your medical aid for (possible) reimbursement.
